Education and research on geriatric mental health. Provides education to students of various disciplines as well as service providers in the community. The program's educational service and clinical practice is rooted in knowledge gained from research. more |
Mount Sinai Hospital – Cyril Reitman Centre for Alzheimers Support and Training for Family Care Givers |
A program targeted to elderly caregivers living at home and managing dementia. They are assessed fully and treated for whatever is necessary and are eligible to enter the Reitman program. The staff are multidisciplinary and the interventions are a blend of individual and group psychotherapy +/- medication as necessary. The defining feature however, is the focus on a specialized population with specific geriatric needs. In addition, the care recipients are part of the intervention in a separate group format. They can be fully assessed and managed in the separate outpatient memory program and join the Reitman dementia group program if appropriate. more |

U-First |
Internet-based course designed specifically for front-line caregivers on how to better understand and care for people with Alzheimer Disease and other dementias. Please note these modules are not a replacement for the formal U-First! Program. They are intended as a supplementary program to reinforce key messages and reach a larger number of front-line workers. It is strongly encouraged and key to the success of the integration of the education into practice that the supervisors of front-line staff access the U-First! program themselves in order to support workers and address organizational issues. more |

Knowledge Exchange
Alzheimer Knowledge Exchange |
A dynamic source of information includes printable materials, Community of Practice, on-line chats and webcasts. The Alzheimer Knowledge Exchange (AKE) supports those interested in improving the Ontario system of care for persons with Alzheimer Disease & Related Dementias (ADRD). It facilitates the exchange of information, innovation and expertise among caregivers, educators, researchers and policy-makers by connecting people to knowledge expertise; connecting people to resources; connecting people to ideas; and connecting innovations to practice. Please browse the links for access to resources, people and ideas, dialogue and exchange opportunities, access to a knowledge broker and a collection of innovations in practice. The AKE has close ties to the Seniors Health Research Transfer Network (SHRTN). more |
Community of Practice (CoP) – Mental Health, Addictions and Behavioural Issues |
Ontario’s Geriatric Mental Health Outreach Teams (GMHOTs) provides facilitation and leadership for this Community of Practice. Across the province, GMHOTs are the major resource for transforming the health system in regards to seniors with serious mental health, addictions and behavioural issues (e.g. responsive behaviours). As a cornerstone to this CoP, GMHOTs have the ability to create a multiplier effect given their mandate for clinical service delivery across and within various sectors of care (e.g. LTCHs, community, etc), knowledge exchange and transfer (e.g. building capacity of care partners) and developing local, regional and provincial partnerships and linkages. more |
NICE - National Initiative for the Care of the Elderly |
Networking and knowledge transfer. It helps get good research into practice. NICE has three main goals
1. Help close the gap between evidence-based research and actual practice
2. Improve the training of existing practitioners, geriatric educational curricula, and interest new students in specializing in geriatric care

Seniors Health Research Transfer Network (SHRTN) |
Knowledge exchange through Community of Practice, library service, knowledge brokers, etc. SHRTN is a province-wide knowledge exchange network that links caregivers with researchers, and policy makers who work together to improve health care for seniors. Membership is free and open to anyone interested in SHRTN’s resources and knowledge: Communities of Practice, Library Service, Knowledge Brokers, Local Implementation Teams, and Access to Technology. more |
